DALE ROSA EQUINE STUDENT & VISITOR FILES

This form is required for any and all individuals who enter property owned by Daniel Widick and Whitney Widick, doing business as Dale Rosa Equine, regardless of participation or non-participation in equine activities. This includes, but is not limited to, parents, legal guardians, students, riders, siblings of participants, grandparents, other family members, friends, guests, and visitors. Any person stepping onto the property must complete and sign this form prior to entry. Participation in equine activities is not required for this requirement to apply.  

All new students must complete this form prior to first lesson sessions. It includes program information, rules & expecations.
